Office of Student Conduct

Welcome to the Office of Student Conduct! Our staff is available to provide assistance and student support during regular business hours: Monday - Friday from 7:45 am to 4:30 pm. Our physical offices in QLC 207 are temporarily closed, so drop-in services are currently not available. However, staff can be reached via email (osc@hawaii.edu), telephone (808-956-4416) or STAR Balance to schedule an appointment. All appointments will be conducted via telephone or zoom. Even though our doors are closed, we are here for you!

COVID-19 Incident Reporting Form

The Office of Student Conduct has developed an online form for UHM employees who wish to report incidents in which they believe COVID-19 guidelines were violated.

Submission of a report will initiate an investigation and review of the incident. Employees who submit reports will receive a confirmation of the submission. If you have any questions, please contact the Office of Student Conduct (osc@hawaii.edu).

Academic Dishonesty Reporting Form

Report an incident of academic dishonesty (cheating, plagiarism, etc.)

How to utilize the Academic Dishonesty Reporting Form:

  • You must have a valid UH account.
  • You must be reporting an incident of academic dishonesty for a class being offered at the Mānoa campus.
  • Clicking on the link above will direct you to the UH "Web Login Service," where you will be prompted for your UH username and password. Once authenticated, you will be able to access the Academic Dishonesty Reporting Form.
  • Should you have any questions or encounter any problems, please contact the Office of Student Conduct for further assistance.
