Room Reservation Request Form

A minimum of two (2) business days is required to process reservations. If you are requesting a room within 2 business days, please call 956-9759.

Before you begin: please read our QLCSS Conference Room Policy (required).

I, undersigned, understand the rules and regulations for reserving a room in the Queen Liliʻuokalani Center for Student Services (QLCSS), and agree to abide by them.*
I also understand that I will be responsible for any loss or damages to the facility.*





Submitting this form is a request only and does not guarantee a reservation. A confirmation e-mail will be sent to the requesting e-mail address usually within 48 hours. (Refer to Requesting Use of a Conference Room.)

A copy of your request will be sent to the requesting e-mail address upon submission of this form.

Please notify the room scheduler of any changes made to your original request, including cancellation of event.
